The information above is reported accurately to the best of my knowledge. I understand that failure to report accurate information with regards to jobs that qualify for prevailing wage under AB219 will cause Folsom Ready Mix to report inaccurate details to government agencies and other participating 3rd parties. I agree that any errors resulting from inaccurate information disclosed above will result in additional fees for the qualifying job to cover additional costs incurred for correcting official government records.
